All course registrants must submit copies of the following documents prior to participation:

We encourage paperwork to be completed and submitted ahead of the class date, but will allow participants to arrive early to complete paperwork, if needed.

  • A copy of a United States driver’s license, government ID, or proof of residency
  • Firearms Certification Statement
  • Participant Waiver, Liability Release, and Assumption of Risk Agreement

By registering for any training, class, or private instruction offered by CIV1 DEFENSE, each registrant must agree and attest to the following:

  • All information provided to CIV1 DEFENSE is true and accurate, to the best of my knowledge and belief.
  • I will be 18 years or age (21 years of age for CCW classes) at the time of the firearms training course for which I am registering OR I will be accompanied by a parent or legal guardian.
  • Upon arrival to the firearms training, I will be required to positively identify myself as the same person certified on the registration.
  • I will sign a waiver releasing CIV1 DEFENSE from any and all claims that may arise from my participation in the firearms training course.
  • I do not have any physical injuries or restrictions that would limit or prevent me from participating in this training, and I will inform CIV1 DEFENSE upon arrival to the training of any change to this attestation.
  • I certify and attest that upon the commencement of the training class, I will not be under the influence of alcohol or any substance, either prescription or over the counter, that affects my judgment.  I understand that the instructors and staff of CIV1 DEFENSE have full authority to prevent or remove me from any training should they suspect that I am not in full control of my faculties, for any reason.
  • I acknowledge that I have received instruction on safety and will comply with any and all rules, regulations, and directions, either written or oral, that I receive from the instructor and/or his or her designated agents.
  • My instruction may be terminated at any time during the training course if my conduct is not deemed satisfactory, at the sole discretion of the instructor or staff.
  • I certify that I am not prohibited, restricted, or limited from owning and/or using firearms by the Federal Government or the State of Florida, for any reason, whatsoever.

Firearms Certification Statement

This Firearms Certification Statement must be reviewed and signed prior to participation in any firearms training class presented by CIV1 DEFENSE.  This statement helps to ensure that no firearms are provided to any participant  in contravention of any existing law restricting the possession of firearms to individuals within the State of Florida.

I understand that persons meeting any of the following conditions are prohibited by Federal or State law to possess a handgun:

  • A felon
  • An illegal immigrant
  • A person under the age of 18, but this shall not apply to the issue of handguns to members of the Armed Forces of the United States, active or reserve, National Guard, State Militia, or R.O.T.C., when on duty or training or the temporary loan of handguns for instructions under the immediate supervision of a parent or adult instructor
  • Those currently charged for a crime that would, upon conviction, bar you from possessing a handgun
  • A person who was dishonorably discharged from any of the armed forces of the United States
  • A person who has been convicted of a crime of violence in any court of the United States, the several states, commonwealths, territories, possessions, or the District of Columbia or who is a fugitive from justice or a habitual drunkard or a drug addict or who has been adjudicated mentally incompetent
  • A person who by order of a circuit judge or county court judge of this State has been adjudged unfit to carry or possess a firearm
  • A person subject to a court order prohibiting stalking, harassing, or threatening an intimate partner or child of an intimate partner or placing them in reasonable fear of bodily injury
  • A person found by a court or mental facility to have certain mental disabilities
  • A fugitive from justice
  • A person who has formally renounced U.S. citizenship
  • A member of a subversive organization

I certify that I am not a person prohibited under Federal or State law from possessing a handgun, and none of the above conditions apply to me.

PARTICIPANT WAIVER, LIABILITY RELEASE, & ASSUMPTION OF RISK AGREEMENT

I, (PARTICIPANT NAME), hereby affirm that I am aware that firearms training and operations have inherent risks which may result in serious injury or death.  I understand that operating firearms with explosive components involves certain inherent risks which may require transport to a medical facility; explosive detonations, misfiring, hang firing, and other malfunctioning of ammunition, the firearm of its components; ricocheting of projectiles, or impacted materials which may occur during training.  I understand that the activities which are necessary for training and for certification, may be conducted at a site that is remote, either by time or distance or both, from such a medical facility. I still choose to proceed with such instructional activities in spite of the possible absence of a medical facility in proximity to the training site.
I understand and agree that neither CIV1 DEFENSE, nor its instructor(s), nor the facility at which I receive my instruction, nor any of their respective employees, officers, agents, contractors, or assigns (hereinafter referred to as “Released Parties”) may be held liable or responsible in any way for any injury, death, or other damages to me or my family, heirs, estate, or assigns that may occur as a result of my participation in this firearms class of as a result of the negligence of any party, including the Released Parties, whether passive or active.
In consideration of being allowed to participate in this course, I hereby personally assume all risks of this course, whether foreseen or unforeseen, that may befall me while I am a participant in this course, including but not limited to the academics, travel, or practical firing activities.
I further release, exempt, and hold harmless said course and Released Parties from any claim or lawsuit by me, my family, estate, heirs, or assigns, arising out of my enrollment and participation in this course, including both claims arising during the course or after I receive my certification.
I also understand that shooting and participating in live fire exercises are physically strenuous activities and that I will be exerting myself during this firearms training course and that if I am injured as a result of a heart attack, panic, hyperventilation, or any other cause, that I expressly assume the risk of said injuries and that I will not hold the Released Parties responsible for the same.
I agree that the use of my image in any format, without compensation, acquired during the class may be used for any legal purpose relating to the advertising and promotion of the CIV1 DEFENSE firearm training program and courses taught by its instructor(s), or in any legal proceeding, whatsoever.
I further state that I am of lawful age and legally competent to sign this liability release, or that I have acquired the written consent of my parent or guardian.
I understand that the terms herein are contractual and not a mere recital, and that I have signed this document of my own free act and with the knowledge that I hereby agree to waive my legal rights.  I further agree if any provision of this Agreement is found to be unenforceable or invalid, that provision shall be severed from this Agreement.  The remainder of this Agreement will then be construed as though the unenforceable provision had ever been contained herein.
I, (PARTICIPANT NAME), BY THIS INSTRUMENT AGREE TO EXEMPT AND RELEASE CIV1 DEFENSE, ITS INSTRUCTORS, THE FACILITY THROUGH WHICH I RECEIVE MY INSTRUCTION, AND ALL RELATED ENTITIES AS DEFINED ABOVE, FROM ALL LIABILITY OR RESPONSIBILITY WHATSOEVER FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH HOWEVER CAUSED, INCLUDING, BUT NOT LIMITED TO, THE NEGLIGENCE OF THE RELEASED PARTIES, WHETHER PASSIVE OR ACTIVE. I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS PARTICIPANT WAIVER, LIABILITY RELEASE, AND ASSUMPTION OF RISK AGREEMENT BY READING IT BEFORE I SIGNED IT ON BEHALF OF MYSELF AND MY HEIRS.
