All course registrants must submit copies of the following documents prior to participation:
We encourage paperwork to be completed and submitted ahead of the class date, but will allow participants to arrive early to complete paperwork, if needed.
- A copy of a United States driver’s license, government ID, or proof of residency
- Firearms Certification Statement
- Participant Waiver, Liability Release, and Assumption of Risk Agreement

By registering for any training, class, or private instruction offered by CIV1 DEFENSE, each registrant must agree and attest to the following:
- All information provided to CIV1 DEFENSE is true and accurate, to the best of my knowledge and belief.
- I will be 18 years or age (21 years of age for CCW classes) at the time of the firearms training course for which I am registering OR I will be accompanied by a parent or legal guardian.
- Upon arrival to the firearms training, I will be required to positively identify myself as the same person certified on the registration.
- I will sign a waiver releasing CIV1 DEFENSE from any and all claims that may arise from my participation in the firearms training course.
- I do not have any physical injuries or restrictions that would limit or prevent me from participating in this training, and I will inform CIV1 DEFENSE upon arrival to the training of any change to this attestation.
- I certify and attest that upon the commencement of the training class, I will not be under the influence of alcohol or any substance, either prescription or over the counter, that affects my judgment. I understand that the instructors and staff of CIV1 DEFENSE have full authority to prevent or remove me from any training should they suspect that I am not in full control of my faculties, for any reason.
- I acknowledge that I have received instruction on safety and will comply with any and all rules, regulations, and directions, either written or oral, that I receive from the instructor and/or his or her designated agents.
- My instruction may be terminated at any time during the training course if my conduct is not deemed satisfactory, at the sole discretion of the instructor or staff.
- I certify that I am not prohibited, restricted, or limited from owning and/or using firearms by the Federal Government or the State of Florida, for any reason, whatsoever.
Firearms Certification Statement
This Firearms Certification Statement must be reviewed and signed prior to participation in any firearms training class presented by CIV1 DEFENSE. This statement helps to ensure that no firearms are provided to any participant in contravention of any existing law restricting the possession of firearms to individuals within the State of Florida.
I understand that persons meeting any of the following conditions are prohibited by Federal or State law to possess a handgun:
- A felon
- An illegal immigrant
- A person under the age of 18, but this shall not apply to the issue of handguns to members of the Armed Forces of the United States, active or reserve, National Guard, State Militia, or R.O.T.C., when on duty or training or the temporary loan of handguns for instructions under the immediate supervision of a parent or adult instructor
- Those currently charged for a crime that would, upon conviction, bar you from possessing a handgun
- A person who was dishonorably discharged from any of the armed forces of the United States
- A person who has been convicted of a crime of violence in any court of the United States, the several states, commonwealths, territories, possessions, or the District of Columbia or who is a fugitive from justice or a habitual drunkard or a drug addict or who has been adjudicated mentally incompetent
- A person who by order of a circuit judge or county court judge of this State has been adjudged unfit to carry or possess a firearm
- A person subject to a court order prohibiting stalking, harassing, or threatening an intimate partner or child of an intimate partner or placing them in reasonable fear of bodily injury
- A person found by a court or mental facility to have certain mental disabilities
- A fugitive from justice
- A person who has formally renounced U.S. citizenship
- A member of a subversive organization
